Acceptable change of name documents

-Change of Name Certificate issued by the Registrar of the relevant Births, Deaths & Marriages  (now the Justice Department)

-An amended Birth Certificate (with notations) issued by the Registrar of the relevant Births, Deaths & Marriages (now the Justice Department)

-Marriage Certificate issued by the Registrar of the relevant Births, Deaths & Marriages (now the Justice Department) (not a wedding certificate issued by the celebrant)

-Divorce papers Decree Nisi or Absolute is acceptable providing it shows the name being reverted to.
